Help for Living with Diabetes
If you or someone you care about has diabetes, Swedes offers a number of helpful resources. We offer diabetes and nutrition education along with support services for patients and their caregivers.
Diabetes Self-Management and Education
At this center, in collaboration with physicians, certified educators and registered licensed dietitians will teach you skills to manage and take control of diabetes and live a healthier, happier life.
Individual and group sessions are offered by certified diabetes educators, including advanced practice nurse, registered nurses and clinical dietitians. Here you’ll learn about day-to-day self-care, including medications, treatment of emergencies, blood glucose monitoring nutrition and exercise, as well as special needs for skin, foot, eye and dental care. We provide initial education upon diagnosis, as well as follow-up education. Ambulatory insulin and pump therapy can also be provided. Appointment times are flexible. If you’re interested, please ask your primary care provider for a referral to the center.
