Request Your MySwedes Chart Account
Anyone 12 or older can request an account. Just ask your healthcare provider for the MySwedes Chart Request for Access Form or download one from this page. If you’re between the ages of 12 and 17, you’ll need to complete the Request for Access for Adolescents Form with your parent or legal guardian.
Parents can request proxy access for minor children (age 11 years and younger). You’ll need to request access in person at your healthcare provider’s office. Access automatically terminates on the child’s 12th birthday. Parents also must have their own MySwedes Chart account to be granted Proxy access.
