SwedishAmerican Home Care Receives Top Quality of Patient Care Star Rating

SwedishAmerican, a division of UW health, is pleased to announce that SwedishAmerican Home Care has received a 4.5-star (on a scale of 1 to 5) Quality of Patient Care Star Rating on Home Health Compare (HHC), a Web site established by the Centers for Medicare & Medicaid Services that consumers can use when choosing a home health care provider.

The Quality of Patient Care Star Rating is based on Outcome and Assessment Information Set (OASIS) assessments and Medicare claims data, and is calculated using the following nine quality measures:

1. Timely Initiation of Care
2. Drug Education on all Medications Provided to Patient/Caregiver
3. Influenza Immunization Received for Current Flu Season
4. Improvement in Ambulation
5. Improvement in Bed Transferring
6. Improvement in Bathing
7. Improvement in Pain Interfering With Activity
8. Improvement in Shortness of Breath
9. Acute Care Hospitalization

SwedishAmerican Home Care's ranking places it in an elite group nationwide. Based on data from OCS HomeCare, reported in the February 8, 2016, issue of Home Health Line, only 11.5 percent of U.S. home care agencies received a ranking of at least 4.5 stars.

"Achieving this score required a team effort, and we are proud of this accomplishment," says SwedishAmerican Home Care Director Tonya Gray. "Although we are proud of our score, we continue to take steps to evaluate our data and enhance clinician training to improve outcomes."

The Quality of Patient Care Star Ratings were first published on HHC in July of 2015 and are updated quarterly. More information is available at: https://www.cms.gov.
