Welcome to MySwedes Chart, SwedishAmerican Health System’s free, online patient portal, available to all SwedishAmerican patients.
MySwedes Chart provides personalized and secure online access to portions of your medical record. It enables you to use the Internet to help manage and receive information about your health from the comfort of your home or office, 24 hours a day.
With MySwedes Chart you can:

Request a MySwedes Chart Account
If you are a patient of one of our physician practices, you will be receiving information about using MySwedes Chart at an upcoming office visit or you may ask a nurse prior to being discharged from the hospital.
Ask your provider’s office for the MySwedes Chart Request for Access Form. You will need to provide the following information:
Your request form will be processed within 3–5 business days and an access code will be sent to your email address. Once you receive your code, you may log on and create a new MySwedes Chart user ID and password, which you will then use to log on to MySwedes Chart.

Proxy access allows parents of minor children (age 11 years and younger) access to sections of their child’s health information records. Access automatically terminates on the child’s 12th birthday. Parents also must have their own MySwedes Chart account in order to be granted Proxy access.
To obtain Proxy access, you will need to request access in person at the clinic. A Request for Child Proxy form must be completed and signed before access will be granted. Patients over age 18 may obtain these forms through their doctor’s office, or viewing and printing the MySwedes Chart Request for Child Proxy Form.
When NOT to use MySwedes Chart
Please do not use MySwedes Chart to send any messages requiring urgent attention. For urgent medical matters, contact your doctor’s office or dial 911.
